摘要:该术式是显微镜结合管道系统行腰椎间盘切除术,全身麻醉,俯卧位,先在C臂定位确定切口,切口取中线旁开0.5cm行直切口(髓核突出侧),长约3cm,经切口中点向内侧倾斜15°~ 20°插入定位针,再次C臂确认准确后,依次沿定位针逐级置入METRx系统(美国Medtronic公司)扩张导管、扩开肌肉纤维,选择适宜深度的工作套管并置入手术区,并与蛇形臂连接固定于手术床上。置手术显微镜,镜下行髓核切除。
Introduction: The discectomy was performed under the microscope combined with the minimal exposure tubular retractor (METRx) system, under general anesthesia and prone position. At first, use C-arm positioning to determine the skin incision. A straight incision was done 0.5 cm near the midline (herniated side) and was about 3cm long. At the incision point, insert the positioning needle leaned 15-20°to the inside. After confirmed by C-arm again, METRx system (Medtronic, US) was implanted step by step along the positioning needleto dilate ducts and open muscle fibers. Choose appropriate depth of casing, put it into operation area, connect with the serpentine arm, and fix it into the surgical bed. Finally place the surgical microscope for lumbar disc resection.
